Job Description:

A nonprofit services organization in the Bronx is actively seeking a compassionate and dedicated Case Manager to join their staff. In this role, the Case Manager will provide case management services to formerly homeless individuals, some with multiple disabilities, in order to help the individual transition and adjust to permanent housing.

Responsibilities

The Case Manager will:

• 
  
• Oversee case coordination for a caseload of 15, including home visits in clients apartments, and provide escort to appointments as needed
  
• Conduct an initial and ongoing assessment of clients , including completion of biannual assessments and psychosocial assessments, and provide crisis intervention when needed
  
• Connect clients to an array of services as appropriate
  
• Maintain record keeping in compliance with agency standards as well as those of City and State agencies
  
• Support group facilitation, including planning and participating in events such as holiday parties, annual BBQ, etc.
  
• Perform other related duties as assigned
  
  
  Qualifications
  
  
  • 
    
  • Associate's or Bachelor's degree
    
  • 2 years of experience working with people with co-occurring disorders
    
  • Proficiency with Microsoft Office suite and databases
    
  • Knowledge of issues of the homeless population
    
  • Knowledge of social service systems
    
  • Superior organization skills
    
    
    Desired skills
    
    
    • 
      
    • Training in Harm Reduction, Housing First, Motivational Interviewing, and Crisis Intervention
      
    • Ability to speak Spanish
