Job Description:
The Case Manager will provide case management support for
patients, including conducting assessments, developing care plans,
and coordinating with healthcare and social services providers to
meet patient needs.Requirements:Candidates must have:Bachelor's
DegreeRequired New York State License to practice as a Social
Worker (LCSW, LMSW, LMHC, CASAC, etc. preferred)Minimum of 3 years
of experience in relevant case management service deliveryPrior
experience in Health Home model preferred
