Job Description:

A growing healthcare network in Westchester County is currently seeking a licensed Registered Nurse (RN) to join their team on a Part-time basis as their new -Manager -- Case Management.

About the Opportunity:

• 
  
• Schedule: Thursdays, Fridays, and Sundays
  
• Hours: 8am to 4pm or 9am to 5pm
  
  Responsibilities:
  
  The Manager (RN) - Case Management will:
  
  • 
    
  • Perform -the initial comprehensive assessment on admission in accordance with the Care Management Department policy
    
  • Screen -all patients by utilizing established tools for high risk indicators (i.e. AIMS, Geisinger, polypharmacy)
    
  • Ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission and assess all populations for potential discharge planning needs
    
  • Provide -ongoing reassessment of needs throughout the hospital course
    
  • Perform utilization review -activity per department policy
    
  • Handle other duties, as needed
    
    
    Qualification:
    
    
    • 
      
    • 5+ years of experience in a Clinical Emergency Department, Critical Care, and/or Medical-Surgical role/setting
      
    • Bachelor's Degree in Nursing
      
    • NYS Registered Nurse (RN) license
      
    • Previous experience as a Case Manager
      
    • Experience in use of Milliman/InterQual criteria
      
    • Computer savvy
      
      Desired Skills:
      
      
      • 
        
      • Master's Degree in Nursing
        
      • Certified Case Manager (CCM) certification
        
      • PRI/Screen certification
        
      • Previous experience in Case Management
        
      • Knowledge of Healthcare Financial, Regulatory, and Payer issues
        
      • Knowledge of State, Local, and Federal programs
