Description:The Government Programs Associate will work with
medical practices to facilitate quality improvement activities
through on-site and remote technical assistance that support
Population Health Management, and Data-driven Informed Decision
Making. This position requires the Government Programs Associate to
coach his or her assigned practices to improve patient outcomes by
developing skills in process improvement, team-based care,
encouraging patient engagement, and analyzing quality data and
measurements. Responsibilities
- Provides technical assistance and support to enrolled practices
to build capacity to progress through program milestones and
goals.
- Oversees timely and successful execution of transformation
model through tracking and reporting on practices' progresses,
develops remedial action plan for at-risk practices.
- Provides supervision and practice performance evaluation
through continuous monitoring and analysis of transformation
program requirements.
- Prepares reports regarding practice performance analysis and
develops strategies with the clinical practices on setting actions
and goals to improve cost, quality and patient experience.
- Provides education to providers on promoting quality and
cost-effective outcomes for their general patient population and
identified care management patients.
- Identifies potential barriers and challenges encountered in
fulfilling the work scope of the practice and implements improved
processes into workflow.
- Develops training goals on the use of data to improve practice
performance while setting clear deadlines for the practice to
follow.
- Provides group coaching or one on one to practice staff and
care team on program related changes and updates through regularly
facilitating meetings with practice to monitor, present and discuss
progress towards program requirements.
- Supports practice implementation of care coordination and care
management initiatives.
- Coaches practices using elements of the EHR, population health,
and data-driven decision making to achieve goals and outcomes;
monitory progress and adjust as needed.
- Engage/assist practices with other healthcare quality
initiatives to improve healthcare outcomes.
Requirements:Qualifications
- BA or BS degree in psychology, nursing, liberal arts,
education, public health, or a related field with relevant skills
to work with professionals in the healthcare field.
- Preferred experience in the healthcare field, but not
required.
- Excellent verbal/written communication, interpersonal,
analytic, and customer service skills, must be a team player with
strong work ethics.
- Manage various projects with detailed project schedules and
work plans.
- Ability to work effectively on simultaneous projects with
competing priorities
- Ability to build productive relationships with clinicians and
internal staff. Will need to train or teach clinical and
non-clinical staff; must be comfortable in giving oral
presentations.
- Ability to work independently and in a team environment.
- Demonstrated skills and experience in negotiation, problem
identifications, and resolution.
- Computer literacy, preferred experience with EHRs
implementation and ability to build reports using Excel.
- Able and willing to travel locally. Benefits
