Job Description:
Eastchester Rehabilitation and Health Care Center -
BROAD FUNCTION:
Assists with the planning, organization, development, coordination
and direction of the Nursing Department in its programs and
activities in accordance with current Federal, State and Local
guidelines and regulations. Follows accepted standards of practice
and implements all such approved activities over a 24 hour period
on the unit to which the RN Unit Manager is responsible. Follows
all established policies and procedures and acts as a role model to
staff regarding mission and policies of the facility. Serves as a
role model in professionalism, conduct, appearance and continuous
improvement. Makes independent decisions as necessary.

SPECIFIC TASKS:
1. - - - - - - - - - Coordinates the planning, implementation, and
evaluation of unit specific resident care/ services on a 24 hour
basis.
2. - - - - - - - - - Provides ongoing in-service education to unit
staff as identified to be needed and documents accordingly on
in-service education record form.
3. - - - - - - - - - - Schedules meetings for nursing and ancillary
staff to discuss goals and problems in order to promote better
nursing care and strengthen staff relationships and rapport.
4. - - - - - - - - - - Facilitates brief morning meetings with
staff after receiving 24 hour report.
5. - - - - - - - - - Participates in physician rounds, team
conferences, special care meetings, and in - services as
assigned.
6. - - - - - - - - - Assigns staff to in-service programs based on
education calendar and in a manner.that ensures adequate unit
coverage.
7. - - - - - - - - - Performs required charting according to policy
and procedure and monitors for similar performance by licensed
nurses over 24 hour period.
8. - - - - - - - - - Implements progressive disciplinary action in
the forms of verbal/written counseling and written warnings
regarding staff job performance and infractions of facility policy
and procedure.
9. - - - - - - - - - Meets with Nursing Administration and union
representatives when necessary to resolve nursing staff grievances.
Will participate in decisions regarding probation, hiring and
disciplinary action up to and including termination as
requested.
10. - - - - Attends meetings with Nursing Administration as
scheduled and participates in policy development when
appropriate.

11. - - - - - Conducts performance evaluation of units' 24-hour
nursing staff yearly and as needed.

12. - - - - - Keeps physidan, family and Nursing Administration
apprised of resident problems/concerns as appropriate.
13. - - - - Monitors staff compliance with facility policy,
procedures.

14. - - - - Provides opportunities for professional growth and
development for unit nursing personnel through recommendation to
community-based seminars and promotion of recognition and
scholarship.
15. - - - - Serves on committees as designated.
16. - - - - - Collaborate with the Medical Staff in planning
resident care, treatment, rehabilitation, etc. as assigned.
17. - - - - Responsible for conducting QA audits of staff
performance and records to validate compliance.
18. - - - - Directs and supervises the units Nurses and CNAs who
cover the unit over 24 hours.
19. - - - - - Provides direct care to residents and/or assists
other staff members with care as needed.
20. - - - - Verifies that physician orders are accurately and
thoroughly transcribed and carried out.
21. - - - - - Maintains integrity of central line dressing changes
and monitoring.
22. - - - - Monitors the physical environment of the unit to ensure
the safety and well-being of residents and staff.
23. - - - - Assures that all emergency supplies, emergency boxes
and suction machines/equipment are in place, stocked with
non-expired items and ready to use.
24. - - - - Reports changes in resident's conditions to physician
and develops an appropriate plan of care.
25. - - - - Establishes and verifies PAR levels of central supplies
required to meet unit/resident needs.
26. - - - - - Oversees unit staff assignments and monitors for
satisfactory completion.
27. - - - - - Attend/lead CCP team meeting collaborate with team
and develops/updates CCP accordingly.
28. - - - - Makes frequent rounds on all residents to ascertain the
resident's condition, quality of care, environmental problems,
complaints, and application of plan of care.
29. - - - - Observes the dining room during resident meal time and
verifies that staff are present and performing their dining room
assignment.

30. - - - - - Monitors and/or observes resident's eating abilities,
adjusts care plan and communicates with interdisciplinary (IDT)
team as appropriate.

31. - - - - - Receive and give report at change of shift and
conveys resident information to the staff when giving
assignments.
32. - - - - Reports and documents all unusual
occurrences/emergencies to the Supervisor/Assistant Director of
Nursing/Director of Nursing or designee.
33. - - - - Handles emergencies through appropriate directives to
unit staff and/or communicates events to management.
34. - - - - Participates/performs audits in the Quality Assurance
Program through completion of audits and development implementing
corrective actions.
35. - - - - Initiates accident/incident reports and conducts
investigations as warranted.
36. - - - - Assess residents that have had accidents/incidents and
review/revise care plan as appropriate.
37. - - - - - Reviews nursing documentation and interfaces with
licensed nurses regarding documentation short-falls.
38. - - - - Initiates admission assessment on new resident,
readmits, and hospital returns and initiates initial care plan.
39. - - - - - Facilitates the nursing aspects of discharge
planning.
40. - - - - Maintains and verifies the confidentiality of all
resident information as per HIPAA guidelines.
41. - - - - - Ensures the reference material maintained at the
nursing stations is current and readily accessible to all
staff.
42. - - - - As assigned by DNS/ADNS, will respond to act upon the
pharmacy consultants Drug Regimen Review as it relates to
nursing.
43. - - - - Drug returns & destruction: Oversees the storage,
handling and documentation of drugs for return or destruction.
44. - - - - Includes Rehab Nursing interventions on the resident
care-plan and in C.N.A assignment to ensure provision of
rehabilitation nursing intervention as assessed/planned to be
needed, per resident.
45. - - - - Designs and oversees the units' Rehab Nursing
Program.
46. - - - - Will communicate respiratory care concerns with
Respiratory Therapist and RN.

47. - - - - Conducts inspection of pressure ulcers for status of
wound, resident response to treatment plan and for staff compliance
with plan of care-and will collaborate with wound care nurse as
determined to be necessary.
48. - - - - Participates in ad-hoc Ethics Committee meetings as
assigned.
49. - - - - Will consistently demonstrate compliance with the
facilities code of conduct described in the acronym R.E.S.P.E.C.T.
(Respect, Effective Communication, Sensitivity, Professionalism,
Exceeding expectations, Courtesy, Teamwork).
50. - - - - Will assume other nursing tasks as assigned by the
ADNS.

JOB QUALIFICATIONS:
1. - - - - - - - - - Nursing Degree from an accredited college or
university. Graduation from an approved school of nursing and
experience may be considered in lieu of degree.
2. - - - - - - - - - Registered Nurse with a current New York State
license.
