About Us:Volunteers of America-Greater New York (VOA-GNY) is an anti-poverty organization that seeks to end homelessness in the New York metropolitan area by the year 2050. We focus our continuum of care to address the most prevalent causes of homelessness: (1) lack of affordable housing; (2) unmet health needs, including but not limited to mental health and substance use; and (3) lack of jobs that create wealth. We know that our clients are confronting longstanding barriers to accessing quality housing, healthcare, and employment. Our programs are designed to ensure that our clients do not have to face these challenges alone. Now in our second century of serving those in need, VOA-GNY understands what it takes to heal families and individuals who are in crisis and have experienced trauma. We operate and provide services in emergency shelters, transitional housing, and permanent supportive housing for close to 11,000 men, women and children through 65 programs located in New York City, Northern New Jersey, and Westchester annually. We are one of the largest providers of services to families and individuals experiencing homelessness in the Greater New York area, serving survivors of domestic violence, people living with HIV/AIDS, veterans, and others who have behavioral health or substance use needs. Many of VOA-Greater New York's programs are nationally recognized and meet the rigorous standards of accrediting bodies CARF, COA and the NAEYC. 'Successful applicants must be vaccinated for COVID-19 before their start date with VOA-GNY, unless you have been approved by VOA-GNY for a medical or religious exception to this requirement.' Position: Community Liaison/Health Care Navigator (SSVF610)Summary: SSVF Healthcare Navigators work closely with the Veteran's primary care provider and members of the Veteran's assigned interdisciplinary treatment team. Minimum Qualifications: Bachelor's degree. The Health Care Navigator must possess excellent judgment and have at least two years of experience in a healthcare or social services area of practice. Knowledge of the VA medical system preferred. Excellent communication and written and verbal communication skills. Supervisory Role: This position does not have supervisory responsibilities. Principal Responsibilities:

  • Connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.
  • SSVF Health Care Navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties.
  • SSVF Healthcare Navigators work closely with the Veteran's primary care provider and members of the Veteran's assigned interdisciplinary treatment team to develop relationships with community leaders, VA staff, and other referral networks is required.
  • The SSVF Health Care Navigator serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran, including regular review of care plan goals with the Veteran, conducting regular non-clinical barrier assessments, and providing resources and referrals needed to support adherence.
  • The Health Care Navigator will act as a liaison between the SSVF grantee and the VA or community medical clinic and work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.
  • Perform other related duties as requested. Expected Results of Principal Responsibilities:
  • Conduct benefit screening
  • Link veterans with health care and other benefits
  • Liaise between veteran, the VA and other agencies. Ensure veterans has access to health care services and adhering to healthcare plan.
  • Provide post placement follow up to address any issues that might affect veterans housing
  • When needed accompany veterans to appointments
  • All other related tasks are completed on time and accurately. Work Environment: While performing the duties of the job, the employee will be in an office and residential shelter environment. Physical Demands: While performing the duties of the job, the employee is regularly required to have visual acuity, talk and hear while communicating with peers and/or vendors.
