Job Description:
PURPOSE OF POSITION:Enhancing the child/youth's ability
regardless of disability, to function as part of a caregiver/family
unit and enhance the caregiver/family's ability to care for the
child/youth in the home and/or community.Payrate: $35hrSPECIFIC
RESPONSIBILITIES1. Interact and engage with family/caregivers and
children/youth to offer educational, advocacy, and support
resources to develop family/caregivers' ability to independently
access community services and activities.2. Maintain and encourage
the caregiver /families' self-sufficiency in caring for the
child/youth in the home and community.3. Address needs and issues
of relevance to the caregiver/family unit as the child/youth is
supported in the home and community.4. Educate and train the
caregiver/family unit on resource availability so that they might
better support and advocate for the needs of the child and
appropriately access needed services.5. Direct instruction and
guidance in the principles of the child/youth's chronic condition
or lifethreatening illness.6. Development and updating of service
plans, including obtaining medical consenter signatures.7.
Obtaining consents and enrollment documents for newly enrolled
children/youth.8. Attend monthly waiver service provider meetings
to stay current on practices and protocols.9. Submit progress
notes, service plans, expense logs, and service provider logs
within set forthrequired timeframes.10. Report incident(s) to
supervisor upon discovery.11. Maintain open communication with
other service providers (e.g., Care Manager) to report on
child/youth's progress and identify any potential barriers and
referrals.12. Submit progress notes, service plans, expense logs,
and service provider logs within set forthrequired timeframes.
Progress notes are due within 3 days of service provision so that
approval from supervisor is completed by the 5th business day
following service provision.13. Setting of Service
Provision:Caregiver/Family Supports and Services can be provided in
the home of the youth or a community setting.Minimum
Qualifications: Caregiver/Family Supports and Service, service
providers are minimally required to have a high school diploma,
high school equivalency preferred or State Education Commencement
Credential with related human service experience.Experience working
with children/youth.Bilingual in one or more of the following:
Spanish, Bengali, Urdu and others, may be required depending on
client needsRELATIONSHIP WITH OTHERS Maintains a strength-based
approach in working with children and families.Accepts and applies
constructive feedback from supervisors to enhance the quality of
servicedelivery.Be a team player.Have a strong sensitivity to
cultural differences present among staff and clients within
ourorganization.Possess a strong belief in people's ability to grow
and change; forge a mutually respectfulpartnership with persons
served and their families.Ability to set limits and maintain
helping role of a practitioner and to intervene
appropriately.WORKING CONDITIONSWork is conducted in the community
of the youth being serviced. There are three SCO locations to meet
with supervisor(s) and submit documentation.TRAINING
REQUIREMENETS1. Prior to Service Delivery:OrientationMandated
Reporter Training (Available in Orientation and via the LMS)2.
Within 6 months of hire: (Employee will be enrolled in the below
via the LMS at hire) Personal Safety Safety in the Community
Strength Based Approached o Suicide Prevention Domestic Violence
Signs and Basic Interventions Trauma Informed CareORGANIZATIONAL
STRUCTURECommunity Habilitation providers will report to their
assigned waiver service provider supervisor and/or designee.
