Job Description:

• 
  
• Assessing patient's physical and psychosocial condition throughout the continuum of care
  
• Providing care for patient's pre, intra and post procedures for all vascular procedures the radiology and cardiac population
  
• Developing and communicating patient plan of care reflecting patient issues and proposed interventions to achieve desired patient outcome
  
• Collaborating with patient, family and healthcare team in implementing plan for patient care and daily scheduling and management of cases
  
• Evaluating patient outcomes, effectiveness of plan of care and readiness for discharge and/or continuing care
  
• Revising plan of care as needed and appropriately referring for additional services where indicated
  
• Providing education as indicated after assessing the learning needs of the patient and family
  
• Participating in and support performance improvement and risk management activities
  
• Acting as preceptor to new staff when requested
  
• Being "on-call per department's needs
  
• Education Requirement:
  
• Required: New York State RN License, Primary Source Verification, AHA BLS, IV/Phlebotomy experience, AHA ACLS,
  
  
  
  Knowledge, Skills, Abilities:
  
  The following experience is required: 2 years of radiology experience IV insertion and phlebotomy skill conscious sedation experience
