Job Description Job Description Job Title: Physical Therapist – Home Care (CWR) Location: Wall Township, NJ 07753 Duration: 13-Week Contract Schedule: Day Shift | 5x8 Pay Rate: $60–$62/hour Job Summary "Navitas Healthcare, LLC" is seeking a dedicated Physical Therapist to provide skilled home-based rehabilitation services. The Physical Therapist will perform comprehensive evaluations, develop individualized treatment plans, and deliver goal-oriented therapy to promote patient safety, functional independence, and optimal outcomes. All services are delivered in accordance with physician orders, organizational policies, and regulatory standards. Responsibilities Conduct comprehensive patient evaluations within required timeframes Develop and implement individualized treatment plans with measurable short- and long-term goals Deliver skilled physical therapy interventions per physician plan of care Complete accurate and timely documentation, including progress notes, reassessments, and discharge summaries Ensure documentation supports medical necessity and continued skilled services Coordinate care with physicians, interdisciplinary team members, and caregivers Supervise Physical Therapy Assistants (PTAs), including competency assessments and timely co-signatures Educate patients and caregivers on home exercise programs, safety, and functional mobility Participate in discharge planning throughout the episode of care Manage scheduling updates and respond promptly to secure messages and voicemail Required Qualifications Minimum 6 months to 1 year of Physical Therapy experience Strong assessment and treatment planning skills Proficiency with computer-based documentation systems Excellent communication and interpersonal skills Ability to complete documentation accurately and within required timelines Education Bachelor’s Degree in Physical Therapy – required Doctorate in Physical Therapy (DPT) – preferred Licensure & Certifications Active Physical Therapist License – required BLS (AHA) Certification – required For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
