Job Description:
Description+ Responsible for meeting with youth and family to
develop individual success plans inclusive of the youth and familys
goals, needs and wishes. In addition, the Social Worker can provide
mental health counseling to youth and family if that has been
identified as a needed support.+ Responsible for the assigned
caseload to build proficiency to the model and/or reach model
fidelity, and meeting the documentation requirement+ Provides
mental health related support services to assigned youth and
families, including diagnosis, and treatment; collaborates with NY
Foundling mental health clinic and other agencies to provide
resource and linkages to families, students and clients.+ Attends
coaching and mentoring and group supervision with the supervisor at
the main office, alternate office, or in the community as needed.
Come ready in supervision having planned and prep for supervision+
Works closely with the mentors team to effectively support the
youth and their families.+ Ensures adherence to all agency, City
and State regulations and policies.+ Conduct and reviews intake,
psychosocial, and on-going assessment, and developing treatment
plan. Intakes and psychosocial to be completed within program
timelines and expectations.+ Responsible for timeliness of
documentation and data entry while maintaining confidential client
records, and meeting program deadlines.+ Maintains clear, concise
and accurate clinical approved documentation of treatment efforts
that promote peer and supervisory review and feedback, and that
demonstrates compliance with evidenced based clinical model, OMH,
and meeting our agencys expectation (i.e. Benefit Assignments,
diagnosis, treatment plan, progress notes, and on)+ Assures
documentation (data entry) for contacts, incidents, and reports are
submitted timely (as indicated by program).+ Adopts a strength
focused approach with families, stakeholder, and within the
program/treatment team.+ Supports the treatment team in providing
school and home-based Mentoring and Advocacy program.+ Communicate
advances and barriers experienced with the clients, team, and
stakeholders, including escalating any feedback received regarding
own areas of improvement, to the supervisor+ Responsible in meeting
the scope of work and deliverables of the program, FAP DYCJ and the
model.+ Participates in Coaching and mentoring weekly to 3 times
per month with supervisor as well as monthly workgroups with
supervisor and plays an active role in development a Development
plan that is tracked monthly to Quarterly.+ Understanding of NYC
Education system, review IEP, and setting education plan.+
Maintains communication with supervisor and escalate any incidents
related to clients and/or staff and complies with agency incident
reporting policy.+ Participates in all model program training,
supervision and consultation activities.+ Builds a relationship
with the schools, FAP, and other stakeholder to ensure
collaboration and proactive communication.+ Available 24/7 for the
team and participates in weekend on-call rotation.+ Provides
coverage support as needed to peers/team members at the main
office, community, or other site.+ Able to respond to the needs of
the team and program by shifting schedule to support+ Conducts
presentation at stakeholder meeting on the program and services+
Participates in leadership development, any leadership related
events/seminar and/or funding sources workshops.+ Splits time
between the main office and other offices, community based on
meeting needs and client services needs+ Attends staff meeting and
agency activities.+ Attends required training set by program,
funding source, agency, and continued commitment to professional
development,+ Other duties as assigned. Organizational expectations

concerning direct reports. Regular supervision and performance
evaluation of direct reports.+ Any other duties and/or tasks that
may be assigned.QualificationsEducationRequired+ Master's Level
Degree or better in Psychology+ Master's Level Degree or better in
Social WorkLicenses & CertificationsPreferred+ Licensed MSW+
