Job Description:

1. Motivate clients with substance use disorder to seek and remain in care2. Utilize evidence-based treatments to engage and deliver services to clients through trauma-informed lenses3. Responsible for collecting GPRA data4. Provide social and recovery support services, referrals to treatment for MAT and other infectious diseases5. Assist in establishing linkages to prevention and harm reduction services6. Provide education about life-saving treatments such as overdose reversal medications and fentanyl stripsand distribute prevention and harm reduction resources.7. Train clients and community members to use opioid overdose reversal medications including naloxone8. Assist in developing relationships with community organizations9. Use EHR to document client encounters and track referral outcomes10. Participate in outreach events to represent the agency11. Adhere to confidentiality policy and HIPPA protocol12. Participate in Job Training and Education Programs, Meetings, and Committees as assigned or indicatedincluding the Advisory CouncilHigh School Diploma or GEDLived experiences (SUD)Prior experience working with the target populationAbility to read, write and speak the English languageKnowledge of computer programsExcellent organizational skillsBilingual Spanish requiredNOTE: This description reflects management's assignment of the essential functions.
