Starting Salary: $52,691 Job Type: Full Time, Non-Exempt Work Schedule: Mondays through Fridays from 8:30 am to 5 pm On Call: Mondays through Thursdays, rotates between teams every 4-5 weeks Locations: CUA 2 - 1900 N 9th Street, Philadelphia, PA 19122 CUA 5 - 3300 Henry Avenue, Philadelphia, PA 19129 ABOUT APM & CUA Asociación Puertorriqueños en Marcha (APM) has been helping families achieve their greatest potential since 1970 by providing early childhood education, foster care & adoption, child welfare, mental & behavioral health, community and economic development, housing, community school services, and violence intervention services to the North Philadelphia communities. The Community Umbrella Agencies (CUA) are community-based agencies established to provide case management services and ensure the permanency, wellbeing, and safety of the children, youth, and their families. APM CUA is looking for compassionate and committed social workers who want to create long-lasting impacts in the lives of children and families. APM CUA 2 services the Lower Eastern North region and CUA 5 services the Logan/Olney region. JOB SUMMARY The CUA Outcome Specialist works to support the Case Management Team in helping families achieve Single Case Plan goals and move towards safe, timely case closure. This position involves working closely with all members of the assigned case management team; with children, biological families, and resource families; and with external agencies and partners. Some of the duties and responsibilities are: Function as part of the larger CUA team responsible for achieving the target goals of improving safety, permanency, and well-being of children, youth, and families. Provide home and community life skills education and intervention services to children, youth, and their caregivers toward ensuring family stabilization, reunification, or other permanency. Participate in Family Team Conferences when necessary. Attend Court Hearings as needed but never in lieu of the CUA Case Manager. Conduct regularly scheduled visits to assigned families and to establish and maintain a trusting relationship with families using a strengths-based approach. Work as part of the CUA Support Team toward achieving the objectives as established in the Single Case Plan (SCP). Facilitate safe case closure for in home service cases. Facilitate reunification or other permanency by, assisting the case manager as it relates to permanency opportunities, tracking and managing movement of cases through the permanency process and supporting with court preparation. Supervise visits when necessary. Assist with transportation when necessary Completing referrals and follow-ups as needed.
